Discharge Medicines Service (DMS)

When you're ready to leave hospital, you can take part in our Discharge Medicines Service. This service puts you directly in touch with our pharmacist, who can talk about your medicines with you, review any changes made during your hospital stay, and make sure you feel confident managing your medication at home.

✅ What Happens

  • The hospital notifies us when you are discharged
  • Our pharmacist contacts you within 72 hours of discharge
  • We review all medicines โ€” including any new ones started in hospital
  • We explain what each medicine is for, how to take it, and any side effects
  • We can liaise with your GP if any changes need updating on your records
  • Follow-up support available if you have further questions

❓ Why It Matters

Hospital stays often result in medicines being started, stopped, or changed. Errors and confusion about medication after discharge are a leading cause of re-admission. The DMS reduces this risk by ensuring a smooth, supported transition home.

Ask the hospital to refer you to our DMS before you leave, or contact us directly after discharge.

Disposal of Unwanted Medicines

Our pharmacy provides a safe, convenient and environmentally responsible service for the disposal of unwanted or out-of-date medicines. Never put old medicines in household bins or flush them down the toilet โ€” bring them to us and we'll dispose of them safely and free of charge.

✅ What We Accept

  • Unused or out-of-date tablets, capsules, and liquids
  • Creams, ointments, and gels
  • Inhalers (MDIs โ€” metered dose inhalers)
  • Eye drops and ear drops
  • Patches and suppositories
  • Controlled drug returns (with appropriate documentation)

❌ What We Cannot Accept

  • Sharps (needles, syringes) โ€” use a sharps bin service
  • Aerosols or pressurised containers
  • Cytotoxic medicines (chemotherapy) โ€” contact your hospital

🌿 Why Correct Disposal Matters

Flushing medicines or putting them in household waste causes environmental harm and contributes to antimicrobial resistance. Safe disposal at a pharmacy is the correct and responsible way to dispose of medicines.

👤 Sharps Disposal

If you use insulin or inject other medicines, you can obtain a free NHS sharps bin from us and arrange a sharps collection through your local council.

Simply bring your unwanted medicines to our pharmacy โ€” no appointment needed.
